First off this tutorial is comprised of info from many source cu.com, xda, gurus, cellphone-gurus.com, sdk, and many others. This will give you root and full flash.
Root methods I experimented with
XDA ([Only registered and activated users can see links]) the Frigid's HTC Super Tool v2 works OK but was a different user interface.
That will get you rooted, s-off, recovery, etc.
Now the flash, full flash to cricket for the HTC EVO Design 4G from Sprint or HTC Hero S from US Cellular
I tried a couple methods to write this and maybe help future users. The first method I tried was the professional method using a paid version of CDMA WS. 3.6 and had no issues with the SPC. It worked good. Also used a paid method for MMS. (sold by Rich)
Then I used the following from cu.com and have modified it to be better explained and tweaked a couple things.

install drivers
Download drivers and unzip to a location on your harddrive.
Open dialer and diall ##3424#, and that should put your phone in diag mode.
Connect your phone to computer and let it try to install drivers.
Now go to Start>rightclick My computer> Device Manager and right click Htc diag> Update Driver Software>Browse computer for driver software>Let me pick from a list of Device drivers on my computer>Have disk and browse to the unzip folder on your harddrive and look for (htcdiag in 32 bit) or (HtcUsbMdmV64 in 64 bit) and install drivers
Get Msl
Install Revskill (free version) and go to Menu>Hardware Forensic>Port Utils>Qc + At-cmd
open and look for the port of diag mode phone(This app sopposedly works on all htc devices to change spc to 000000)
Now go to code tab and click on Htc:Unlock spc
Then click on Change Spc and your done with revskill. Close app
program phone
This is the point where you will be using QPST
Install qpst
open qpst configuration
click on add port and add Usb/Qc diagnostic port (your phone diag port)
Click on start Clients>Service Programming
Phone should show up in menu and press ok
Click on read from phone and send spc 000000 press ok
Click on CDMA tab
go to Directory# and put in your phone number.
go to IMSI_S and put your min number
click on roam tab and click on Browse and load your prl file
Click on M.I.P tab and go to User profile 0
[Only registered and activated users can see links]
do the same for Tethered Nai
click on both enter text string bubbles and type cricket on both spaces
MN-HA SPI= 12c
MN-AAA SPI= 2
DMU Public= 10
Do the same for User profile 1
now on M.I.P tab go to Active user and choose 0
on Mobile IP Behavior change to Simple IP only
Click on PPP config Tab
press Um Button
[Only registered and activated users can see links]
do the same for User ID
password put in cricket
now click on AN button
[Only registered and activated users can see links]
and password cricket
Now while in QPST, find the EVRC area, and change all the setting to EVRC (if they are not already) this will prevent the problem of no outgoing calls in some areas.
now click on write to phone and wait for it to reboot you should have (talk, text, and internet working)
MMS
This method I did not have success with and used Rich's app.
install Apn Backup and restore on your phone and astro file manager from the market to your phone
open apn Backup and restore and click on backup apn and save it
open astro file manager and to to the saved xml file in the apnbackupand restore folder and open as test
in the Production part of the apns
find MMS PROXY and put "null" between quotes
do the same for MMS PORT
In the mmsc fill in "[Only registered and activated users can see links]"
now save file
close astro file manager and open apnbackupandrestore again
click on delete apns and delete apns
now click on restore apn and choose the apn you just modified
now open up messaging app and go to settings
scroll down to Maximum message size and choose 500k
If youtube won't load videos
open dialer and dial ##3282#
click on edit and type in 000000 for code
now click on advanced
Scroll down to RTSP/HTTP Settings
Delete RTSP proxy ip and HTTP proxy ip info
and change both ports to zero now backup to main screen and reboot your phone
