CDMA Workshop is what I use to get most of the security locks...otherwise, try Samsung PST with the right DLL...Read General Info Screen...and there you go my friend...

Well...if that Juke is not conecting you might have to put the phone in Bootloader Mode and download the bin file with Samsung Pst....Emergency mode...thats what I would do...i hope that helps.. ;)

How to disable MTP:
Go to 'Settings & Tools'
Press # key
Put '000000' and press OK
Select '3. MTP setting'
Choose 'MSM'
After that, you can go along with CDMA Workshop.

The problem is there is no way to get there because it has user lock.

I have dll and bin,but it doesn't help because samsung pst it doesn't see it.
if i put in Emergency mode samsung pst see it and work, but it doesn't remove the user lock code either.

HAHA that's my bad...ok.
1) You've tried the last 4 of the phone number
2) Tried reuploading the software in Emergency Mode
3) Tried 0000, 1111, 1234 and the last 4 of the ESN/MEID
Are you able to dial out for calls or is that locked out as well? Have you tried reading it in BITPIM as a SCH-A950? That might give you access to the file system even on MTP mode. Probably not, but it's worth a shot.

Are you able to make calls out? Someone suggested that if you are able to call out and the phone hasn't been blacklisted on verizon, that you could activate it as a prepaid and do a *22890 to reset the user lock to the last 4 of the new phone number.
Call out meaning "dial", not necessarily connect a call.

I can't make calls because is not activate and what i want is clear all info off.
i have more than one phone locked and all are with mpt enable.

I am right there with you. How do you get it into the emergency bootloader mode? If I could get into mine I might be able to figure out how to reset that **** user lock.

Try the last 4 digits

there is a memory reset code that you can use but i can't remember the code maybe some other member can remember and its a 50 50 chance and you will lose everything on the phone.
emergency bootloader mode is 9 and power

Yep I did all of that and they changed the user lock code. Does anyone have the firmware/bin so I can just re-flash them? They are going onto cricKet anyway.

I don't remember if this works on the Juke, but Factory Reset could be ##RTN# (Think it's Return to Manufacturer).
